COVID-19 Response 27/03/2020
Our Focused Care Practitioners are very much on the NHS frontline and supporting vulnerable patients and GP Practices during this difficult time. If you are a patient with particular concerns please contact your Focused Care Practitioner directly via the mobile number they have given you, their NHSMail address, or your GP surgery.
However, in order to support the national effort to reduce contact the operations office has moved to home-working.
To contact one of our admin support team please use one of the following methods:
Phone: 07551 157959
Tel: 0161 2043702 (Voicemail only, but we will pick these up remotely and respond as soon as possible).
Please stay safe and well, and we will do our best to support you whatever way we can.
